The horror is Legion.

Kinderman, investigating mysterious murders. Kinderman doesn't believe in demons and possession. Not yet. Jason Miller reprises his priestly role from the original. And look for Samuel L. Jackson, pro basketball's Patrick Ewing and more bit-role surprises.

Better than the second film but not the first - batman71
The Exorcist 3 is way better than the second Exorcist film but not as good as the first film. The film stars George C Scott as a detective on the trail of a serial killer who's killing are very similar to the Gemini killer who was put to death 15 years before. There are some creepy scenes here and some good performances, but oddly enough not from George who seems to be over acting in all his scenes. It's not a must see film but it's not a bad horror film.Great moments (and humour) dot this passable film - AVreviews
Nothing can compare to the original, but this second sequel makes a decent attempt to resurrect characters and plot from the original and provide a new spin. It fails to be, as a whole, a great movie, but it's OK, with several good moments and actors bringing their best.
The confessional confession was a chilling moment. A static shot of a hospital hallway leaves watchers waiting for a sudden scare. The banter and relationship between Kinderman and Father Dyer bring levity.Just Okay. - cathyottawa
I think this got rave reviews because, compared to the giant misfire that was Exorcist 2, this was pretty good.
William Peter Blatty, author of the book the original Exorcist was based on, not only wrote the screenplay for this film, which is based on his novel Legion, but actually directed it as well.
Blatty didn't want this to be a sequel, intending to call it simply "Legion", and that would have made more sense, because the ties to the original are tenuous at best. They bring back Jason Miller to play Father Karras for a few scenes, in one of the more ridiculous "he didn't really die!" cheats I've ever seen (and I guess no one noticed that they buried an empty casket). But that's the sole tie.
The scares here are pretty tame. Unlike the Exorcist, where the scares were very visual, here, the horror is more about description of gruesome crimes, than seeing the crimes themselves. There's still some creepy stuff, no question, but it's not nearly as shocking as the original.
Also, it should be noted that this isn't really about an exorcism. A brief one was inserted (at the insistence of producers), but this showdown between good and evil involves a gun, more than the Bible.
Finally, I think George C. Scott was all wrong for this part. I also wasn't impressed with Blatty the director. For a movie that came out in 1990, this looks very much like a movie from the 70s, and not in a good way.
